Since our founding in 2010 by Dr. Jason Langheier, we have supported over 1.5 million members across more than 700 health plans, employers, and health systems. We have raised over $70 million in funding from prominent investors to help us integrate nutrition counseling with cost-effective meal planning. We are currently looking for a Senior Community Health Outreach Coordinator to join our team as a contractor on an on-site basis in Cook County. What you'll be doing
- You will attend various enrollment events to build awareness for our foodcare programs.
- You will educate diverse populations on topics such as healthy eating, disease prevention, and how to access SNAP benefits.
- You will screen event attendees for program eligibility and help them schedule their nutrition counseling appointments.
What you'll bring
To succeed in this role, you should be a proactive advocate for your community who is comfortable handling sensitive information with discretion. You must be highly organized and capable of maintaining accurate records of your outreach activities. We are looking for someone who is proficient in the Microsoft Office Suite, including Outlook, Word, Excel, and PowerPoint. Because you will be working directly with the community, you must be bilingual in English and Spanish. Additionally, you need to be comfortable traveling to various health and community centers throughout Cook County to perform your duties.
