About the Role

We are hiring a Licensed Practical Nurse for a full-time, remote position. In this mid-level role, you will act as a vital link between patients and their healthcare resources. You will be responsible for managing chronic care needs and ensuring that every patient receives the personalized attention they deserve.

Key Responsibilities

• Proactively manage your patient census by closing gaps in both clinical and non-clinical care.
• Provide essential patient education regarding chronic condition management and health literacy.
• Oversee medication management tasks, including reconciliation, identifying potential concerns, and coordinating necessary refills.

Requirements

To be successful in this role, you should possess the following qualifications and skills:

• An active Michigan LPN license is required.
• At least 3 years of direct, patient-facing nursing experience.
• Strong technical proficiency, specifically with the Microsoft Office Suite.
• Excellent verbal and written communication skills.
• A dedicated, HIPAA-compliant home office space with reliable high-speed internet.

Location

This is a fully remote position based in the United States.

Compensation and Benefits
