We are currently looking for a Senior Community Health Worker to join our team and help us launch an innovative behavioral health initiative that utilizes AI and behavioral economics to improve patient outcomes.
Responsibilities
- Provide educational support to members regarding preventative care, mental wellness, and behavioral health.
- Conduct proactive outreach to high-need members to help them reach their personal health goals.
- Perform thorough needs assessments and assist patients in addressing social determinants of health.
- Coordinate care by making referrals to medical and social resources and acting as a liaison between clients and service providers.
- Maintain accurate documentation of all patient interactions and organize relevant information efficiently.
- Manage relationships with local healthcare facilities and community organizations.
- Assist patients in using the Nuna app and provide direct feedback from the field to our product team.
- Support patients through a mix of in-person visits and virtual interactions via phone or video.
Must-haves
- Master’s degree in Social Work, Public Health, or equivalent professional experience.
- Strong proficiency in using technology for remote support and virtual communication.
- Excellent problem-solving abilities and a creative approach to overcoming healthcare challenges.
- Demonstrated cultural competency and a deep commitment to serving diverse or underserved populations.
- Strong organizational skills and the ability to work effectively in an independent, dynamic environment.
- Familiarity with the Greater Chicago area and its community resources, with the ability to travel within surrounding counties.
- A valid driver’s license and access to an insured vehicle.
- Fluency in English.
Nice-to-haves
- Current Community Health Worker certification.
- Prior experience conducting home visits or working with managed care populations.
- Direct experience working with Chicago Medicaid populations.
- Background in customer or client service roles.
