The opportunity

We are looking for a Senior Community Health Worker to join our team as a foundational hire. This is a unique chance to shape our behavioral health efforts as we deploy an innovative, AI-driven patient experience. You will be instrumental in bridging the gap between physicians and patients, helping individuals adopt healthier behaviors through advanced technology and behavioral economics. As we move rapidly to bring this product to market, you will have the autonomy to build and scale our outreach efforts while working closely with our product team to refine our approach based on your real-world insights.

A day in the life

• You will provide direct support to members, both virtually and in-person, by conducting health assessments, facilitating short-term interventions, and coaching patients to reach their personal wellness goals.
• You will act as a vital liaison between patients and our broader care team, managing referrals for social or medical resources and advocating for the needs of the individuals you serve.
• You will serve as a technology guide, teaching patients how to use the Nuna app and gathering essential feedback from the field to help our product team improve the patient experience.

Who you are

You are a highly motivated professional with a passion for improving health outcomes through technology. You possess a Master’s Degree in Social Work, Public Health, or equivalent experience, and you are comfortable navigating various digital platforms for documentation and remote support. Because this role requires deep ties to the local community, you are familiar with the Greater Chicago area and its resources, and you are willing to travel within the surrounding counties. You are a creative problem solver who thrives in dynamic environments, and you bring strong cultural competency to your work with diverse populations. Proficiency in English is required for this role.
