Care Coordination Specialist - REMOTE/HYBRID ROLE at Florida Medical Clinic

We are currently looking for a dedicated Care Coordination Specialist to join our team at HealthSync in a full-time, hybrid capacity. Our vision is to be the premier choice for healthcare in our community, and we pride ourselves on delivering superior quality service to every patient we serve. This role is based in the Tampa Bay region, and while it will transition to a remote schedule, you will need to be available to work from our Tampa office one day a week. Please note that the position requires an initial 8 to 12 weeks of onsite training at our Commerce Park Boulevard location.

Responsibilities

• Conduct proactive outreach to patients via phone, email, or text to provide guidance and support for our care coordination programs.
• Manage inbound communications and resolve patient needs efficiently while maintaining accurate records in our care management platforms.
• Build and maintain positive, professional relationships with patients, their families, caregivers, and clinical staff to ensure seamless care delivery.

Requirements

• Current Medical Assistant Certification is required for this role.
• At least one year of experience in a similar role, such as customer service or a centralized patient scheduling center.
• Strong proficiency in Medical Terminology and a solid understanding of HIPAA regulations to ensure patient privacy.
• Experience with EHR systems and beginner to intermediate skills in Excel.
• Excellent communication skills, particularly over the phone, and the ability to thrive in a fast-paced environment.
• Bilingual abilities are considered a plus.
