Pair Team is an end-to-end operations platform that connects vulnerable patients to local care teams. Pair Team is on a mission to connect underserved communities to high quality care as a tech-enabled medical group focused on high-risk Medicaid and Duals patients. Their novel whole-person care model - social, clinical, behavioral - is quarterbacked by a multidisciplinary care team across their network of shelters, food pantries, and other community partners leveraging Arc, their proprietary care management platform. Pair Team is one of the largest providers of a high-needs case management benefit in California and on track to build the country's largest clinically-integrated network of community-based organizations.
